As presented during Step 3 (Agreements) of online membership enrollment at legacyhealth.fit/onboarding.html
The checkbox below is the exact SMS opt-in presented to prospective members during the online membership signup flow. It appears on the Agreements step, after the member has entered contact information and selected a plan, and before payment is collected.
The opt-in is entirely optional and unchecked by default. Members must actively check the box to consent to SMS. Consent state is stored in the member database record (sms_opt_in) and verified before any non-emergency promotional SMS is sent.
